Safe Environment Requirements
To volunteer in our RE classes, or be an active paid substitute catechist, we need the following information. Please print and fill out the necessary forms and return to the parish office.
- Basic Criminal Background Check Form - Complete every 3 years. (paid and volunteer)
- Confidential Questionnaire - Complete every year. (paid and volunteer)
- Complete the Safe Environment Training Complete every year. (paid and volunteer)
- FBI Release Form Complete 1 time only. (anyone who is finger printed)
- Finger Printing - Complete 1 time only. (paid staff and anyone who works with children). Please coordinate this with our Safe Environment Coordinators.
